4-H Record Books

A 4-H Record Book is documentation of all of the 4-H activities at the club, county, and state level that a youth has participated in. Many 4-H members will attest to the benefit and importance of Record Books and record keeping as they apply for higher education, prepare their resume, and submit applications for jobs.
A yearly 4-H Record Book is required in order for a 4-H member to qualify for County Level awards, pins, higher education scholarships, or scholarships for awarded trips. Record books are to be reviewed by a club leader and ultimately the Awards and Scholarship Committee prior to qualifying for county level awards, pins, scholarships, or awarded trips.
Why do we do Record Books?
The purpose of 4-H record keeping is to teach how to keep records, which is an important life skill. Record keeping is an ongoing process that does not end when the 4-H year ends.
Objectives:
- To learn how to set goals, how to make plans for action, and self-evaluation.
- To learn the skills of record keeping and organization to use now and in the future.
- To learn to communicate and summarize.
- To learn responsibility by completing a task.
- To evaluate information that will market personal skills through future resumes and application forms.
Cloverbud Participation Form (K5 through 2nd grade)
Cloverbuds create good record book habits by filling out a Cloverbud Participation Form each year and begin their permanent record book in 3rd grade.
Record Books* (Grades 3 and up)
* As of the 2024-2025 4-H year, the Waukesha County 4-H Record Book was updated to an easy to use fillable PDF format. Youth that have actively been using the old Record Book version have the option of switching to the updated fillable PDF format or continuing to use the old version for their remaining 4-H years.

Extension Waukesha County is located in the Administration Center of the Waukesha County Courthouse Complex. Enter through the main Courthouse doors. After going through security, take the corridor on the left to the the Administration Center. Take the elevators or stairs to the ground floor and go to room G22.
